Scoop was located in sushi rice and the handle of the scoop was touching the rice. Shelving and outside of equipment has accumulation of food debris, grease and dirt. The outside of food storage containers have accumulation of dirt, food, debris and grease. Floors, walls and ceiling are soiled with accumulation of dirt, food debris and grease. Exterior door in kitchen is not closed properly.
