Friday is the deadline to sign up for a charity golf tournament benefitting Cumming resident and former Polo Golf & Country Club professional John David Keith.

In 2008, Keith was diagnosed with Amyotrophic Lateral Sclerosis, or ALS. Also known as Lou Gehrig’s Disease, it is a progressive and degenerative disease that affects nerve cells in the brain and spinal column.

The tournament on Monday will also serve as a celebration of Keith, who turns 44 that day, and his mother and caregiver, Renee. They are scheduled to visit the course at 2 p.m.

Check-in for the tournament is set for 8 a.m. at the club, 6300 Polo Club Drive, with a 9 a.m. start.

The format is a four-person scramble. Cost to participate is $90 per player and includes green fees, cart and range balls. There will also be a silent auction.

To register for the tournament, e-mail Harmony Kiser at harmony.kiser@comcast.net or call (770) 205-7899.

Those wishing to show their support can also make checks payable to the John David Keith Medical Relief Fund.

Donations can be dropped off at any RBC Bank location in the metro Atlanta area or mail to: The John David Keith Medical Relief Fund, c/o Harmony Kiser, 6230 Lenbrook Court, Cumming, GA 30040.
