Prep tops and tables have accumulation of food debris. Fryer and wok station have accumulation of food, grease and dirty on the surface. Vent hood has deteriorated seal which is hanging from vent hood above cooking area. Also, vent hood has accumulation of dirt and grease. Queso cooling in ice cooled about 4 degrees in 20 minutes. Advised to cool in thinner containers on ice. Foods being thawed in stagnant water buckets. Wet wiping cloths stored on prep table. Vegetable prep sink used for thawing chicken. Storage shelves in kitchen and in walk-in cooler have food residue build-up on them.

Notes: Employee handling raw fish with double gloved hands removed one pair of gloves and put a new second pair of gloves. No hand washing step was observed before changing tasks. Salmon on menu indicated as being served raw or undercooked with no parasite destruction letter available when requested. Food containers stored as clean still have food residue on them. Foods not reheated to 165 degrees prior to being held hot. Reheating to 165 was not properly performed. Product discarded.
